Vibration sense is one of the first things lost when diabetes damages the large nerve fibres of the foot. Unlike a simple yes/no screen, vibration perception threshold (VPT) testing gives a number, so nerve damage can be tracked over time. The IWGDF 2023 guidelines recognise a raised vibration threshold as a strong marker of feet at high risk of ulceration.
How Vibration Perception Testing Is Done
- A biothesiometer or a 128 Hz tuning fork is placed on the tip of the big toe and other bony points.
- With a biothesiometer, the vibration is slowly increased until you first feel it, and that voltage is recorded.
- With a tuning fork, the examiner notes whether and for how long you sense the vibration.
- A higher threshold means more nerve damage; readings are compared with normal ranges for your age.
- The test is painless and takes just a few minutes per foot.
VPT is most useful alongside monofilament testing and a full neurosensory examination.
